I can't believe I forgot to include one of the best faked agonies I've ever seen on a mainstream TV show - and it was Star Trek: Enterprise to boot. The episode "The Xindi" has a scene in which the show's resident hotti...er...Vulcan,  T'Pol, is undergoing a form of acupressure (if you don't know the show there's no real point in explaining why). The character clearly climaxes from this. I remember seeing it and nearly falling off my chair as it was something I definitely didn't expect to see on a network TV show.
